      <image:title>Mastectomy Alone - Know the facts about scars!</image:title>
      <image:caption>Mastectomy scars vary in size and character. Mastectomy scars typically are oriented horizontally across your chest. Scars take almost a full year to mature to their final form. Usually, scars become more red in the first few months, and eventually become more pale, and flat. In addition to the linear mastectomy scar , you may have a “contour deformity” where the breast tissue was resected, meaning that you may have an indentation or tight/adherent area of scarring. This may be more pronounced if you have had radiation to that breast. (See RADIATION information) Things that have been shown to improve scarring include: Firm scar massage along the direct of the scar Silicone sheet scar therapy Avoid sun tanning or sun exposure to scar for one year. Some patients may develop Hypertrophic (scar that are raised, wide and red), or Keloid (Scar that have a raised shape that extends beyond the borders of the initial scar).       <image:caption>Fat grafting provides a surgical option help with chest contour deformities or a scar that is tight or tethered to the chest wall after surgery. This involves liposuction or removal of fast cells from one area of your body (usually your belly, flanks or thighs), and injecting it under the scarred tissue.       <image:title>Partial Breast Defect - Fat Grafting</image:title>
      <image:caption>This a surgical process where fat is transferred from one body area (e.g. abdomen, thighs, flanks) to the breast in order to improve its contour and balance the chest appearance Fat grafting typically involves liposuction of the abdomen, flank or thighs, and then injection of the fat to areas in the breast that are scarred or require volume. This may be in areas of scar from the mastectomy, from a lumpectomy, or even due to irregular contour after implant or tissue based reconstruction. Not all the fat that in transferred will remain long term - an estimated 50% will remain, although the exact amount is difficult to tell. Fat grafting often requires a few procedures depending on contour and the amount of fat graft “take.” Most often, this is done in the main operating room under a general anesthetic. Sometimes, if only small volumes need to be corrected, this can be done using local anesthetic (freezing). Interesting fact: If you gain weight, the fat cells that have been transferred from your abdomen to your chest will also increase in size!</image:caption>

      <image:title>Implant-based Reconstruction - Two-Stage Implant Reconstruction:</image:title>
      <image:caption>Involves the placement of “tissue expanders” at the time of mastectomy that stretch out the skin and muscle to make room for the placement of an implant at a later date.   At the time of placement, tissue expanders will have some fluid (saline) injected, and approximately 2 weeks following surgery, more fluid will be added through a needle into a port of the tissue expanders. Usually, the expanders are then filled weekly. This process gradually stretches the overlying skin and muscle in order to create a pocket for the future implant. The size of the final implant will be determined by patient preference, and how the skin and muscle respond to the stretch.</image:caption>

      <image:title>Tissue-based Reconstruction - Abdominal Tissue Flap:</image:title>
      <image:caption>Many women elect to have reconstruction using tissue from their own abdomen. Tissue that is moved from one part of your body to another is called a “Flap.” This is often referred to as a T.R.A.M. (Transverse Rectus Abdominis Muscle) Flap or a D.I.E.P. (Deep Inferior Epigastric Perforator) Flap. The only major difference between these two flaps is that the TRAM includes a small piece of muscle, whereas the DIEP is only skin and fat; otherwise they are essentially the same. Surgeons will try to avoid muscle resection, but the decision to move small sections of muscle will be made during the surgery. In both scenarios, the blood vessels from the abdominal tissue are found and attached to blood vessels at the chest under a microscope.</image:caption>

      <image:caption>In some patients, the latissimus dorsi muscle (large muscle on your back), may be moved to your chest. Very often, an implant or tissue expander can be placed below this muscle. This flap requires lifting the muscle, fat and skin from an area on your back and tunnelling it under your armpit to reconstruct your breast.</image:caption>
